As you chain kills together, your streak climbs through tiers, eventually reaching Massacre if you're fast and efficient enough. A timer tracks your momentum, refreshing as long as enemies keep dying from direct damage, damage over time, or fresh kills before it runs out. Higher tiers grant bonus experience and seasonal reputation based on how many enemies you've taken down, but the system is unforgiving—death instantly resets everything. That alone pushes players to rethink safer, slower builds in favor of speed and precision. Whether you're farming Helltides, pushing the Pit, or clearing dungeons, the goal becomes staying in motion and never letting the flow break.
Bloodied Items are where the system really shines. This new item quality can appear on almost any piece of dropped gear and can stack alongside Ancestral or other existing modifiers. What makes them special is that their affixes scale dynamically with your current Killstreak Tier, meaning the stronger your streak, the stronger your gear becomes. Multiple Bloodied affixes stack additively, so wearing several at once can create powerful synergies that snowball as long as you stay alive. These Bloodied affixes are split across different gear slots, each pushing a slightly different playstyle. Weapon affixes are tied to total kills within a streak, triggering effects such as cooldown resets or extended Berserking after hitting certain kill counts, which rewards sustained pressure rather than just hitting the top tier. Jewelry leans into loot manipulation, increasing the chance for elite drops like Runes at higher tiers, letting players tailor their farming toward specific rewards.
To hunt Bloodied gear more directly, Season 12 introduces Bloodied Sigils, available from Torment I and above. These can be used in Nightmare Dungeons, Infernal Hordes, and Lair Boss encounters, increasing difficulty by an extra Torment tier and adding the Relentless Butcher modifier. In these runs, the Butcher actively hunts you and respawns immediately when killed, without replacing normal spawns. That means it's possible to face multiple Butchers at once, turning every run into a constant fight for survival. The reward is guaranteed Bloodied Item drops, making these activities high-risk but extremely tempting for players confident in their builds. PTR players will start with access to Bloodied gear and Sigils, allowing them to test the system right away.
Season 12 also adds five new Uniques that play directly into the Killstreak theme. The Blood-Mad Idol amulet works across all classes, granting near-constant Berserking at the cost of taking Burning damage. Wendigo Brand rewards momentum by stacking damage and maximum life every 15 recent kills. Other Uniques cater to specific class groups, like Rustbitten Dirk for burst damage against isolated enemies, Wyrdskin gloves that dynamically apply Vulnerable or Weakened effects, and the Thousand-Eye Reaver axe, which boosts movement and Ferocity-based playstyles.
